HARMONIZED HEALTH 2.0
The Project
The Harmonized Health 2.0 project implements one of the key recommendations from Harmonized Health 1.0: to establish a simple, effective solution to improving care coordination, collaboration, and integration by placing a Community Care Coordinator as a critical bridge between people, service providers, medical professionals, and other related agencies.
A Community Care Coordinator role offers every person the opportunity to be listened to while being met where they are at and puts greater focus on the 95% of life not spent at appointments. Removing cost as a barrier allows individuals and families to fully participate in this innovative approach to care. Project outcomes are being professionally evaluated.
The project will finish in February 2025.
